Your Personal Information

When visiting this website, no personal data will be processed unless you complete the contact form. If you fill out the contact form, this data will be collected and processed by OGEEPLAST CLINIC. Additional personal information collected through direct contact with you may also be processed and added to your personal patient file. We will maintain the confidentiality of your health information and will only use it for the following purposes:

Processing: We may use your health information to provide you with our professional services. We have established « minimum necessity or need to know » standards that limit access to your health information based on the primary functions of our staff. Each staff member is required to sign a confidentiality agreement.

Disclosure: We may disclose and/or share your health information with other healthcare professionals who provide you with treatment and/or services. These professionals will have a privacy policy similar to this one. Your health information may also be disclosed to your family, friends, and/or other persons you choose to involve in your care, but only if you agree to it.

Emergencies: We may use or disclose your health information to notify or assist in notifying a family member or any person responsible for your care in the event of an emergency regarding your care, location, general condition, or death. Whenever possible, we will give you the opportunity to object to this use or disclosure. In an emergency or if you are incapacitated, we will use our professional judgment to disclose only information directly related to your care. We will also use our professional judgment to make reasonable inferences about your best interest in allowing someone to pick up filled prescriptions, medical imaging, or other similar forms of health information and/or supplies unless you have given us notice otherwise.

Healthcare Operations: We will use and disclose your health information to ensure the operation of our practice. Examples of personnel who may have access to this information include, but are not limited to, our medical records staff, external health or management reviewers, and individuals performing similar activities.

Requirement of Law: We may use or disclose your health information when required by law. (Court orders or administrative orders, subpoenas, discovery requests, or other legal processes). We will use and disclose your information upon request for national security, intelligence, and other federal officials and/or if you are an inmate or under the custody of law enforcement.

This disclosure involves our business office staff and may include insurance companies (private and public) or other companies that may be involved in sending statements and/or collecting unpaid balances.

OGEEPLAST CLINIC takes its role as a healthcare provider seriously and wishes to proactively inform its clients about significant changes in legislation or trends in plastic, reconstructive, and aesthetic surgery that may affect the patient. In accordance with the General Data Protection Regulation, OGEEPLAST CLINIC requests prior consent from each client for sending newsletters. With every form of informative communication, OGEEPLAST CLINIC will always offer the option to withdraw the given consent if necessary.

Right to Object
